Signing up and Payment

Once you have decided on an offering and would like to sign up for it, please review the Privacy Practices and complete the Client Information and Consent Form (also available in the Sessions and Classes section)

To finish signing up, please download, complete and submit the completed Client Information and Consent form via scan and email it to metaphysicalmindbodyspirit@gmail.com. You will receive a response to your submission to arrange an appointment. Please pay for your appointment prior to your session.
